The care around your care

A prescription is one part of your story.

Start with responsibilities

Support is not one service.

Before adding a membership, ask what your current care team already provides.

Prescribing and medical follow-up
A licensed clinician assesses treatment, medication changes and clinical concerns. Confirm who owns this role and how to reach them.
Nutrition and strength support
A registered dietitian can provide individualized nutrition care. Activity support should reflect your needs and professional guidance.
Coaching, tools and community
Apps and coaches can support routines and preparation. They do not replace a prescriber or guarantee a result after stopping medication.

A starting point for research

Compare what the support actually includes.

Three examples of different service models, in alphabetical order. Descriptions come from each provider's public page, read October 18, 2026. We have not used these services, independently verified outcomes or confirmed your eligibility. Inclusion is not an endorsement or a paid ranking.

A filter shows what the sources explicitly describe. A missing label means unconfirmed in this comparison, not that a service is unavailable. These filters do not assess suitability.

Dietitian-led telehealth

Nourish

Separate Companion and Off Ramp nutrition pathways. These are dietitian services, not a substitute for prescribing care.

Published costs
$145 per session at the published self-pay rate. Insurance-based cost and visit allowance depend on your benefits; not a guaranteed $0 service.
Commitment and cancellation
Appointment FAQ: cancel or reschedule at least 24 hours ahead to avoid the stated $75 late-cancellation/no-show fee.
Follow-up and responsibility
Individual dietitian sessions. Ask about visit frequency, messaging response times and communication with your existing prescriber.
Still needs confirmation
State access, clinician credentials, your benefits, visit length and prescriber coordination remain individual checks.

Physician-led transition care

Toward Health

Medication-transition assessment, nutrition, movement, monitoring and coordination with existing medical teams; changes only when clinically appropriate.

Published costs
$500 registration + $500/month for six months = $3,500 initial program commitment. This is not a complete medication/lab quote or a published maintenance price.
Commitment and cancellation
The page says automatic enrollment into maintenance after six months and cancellation flexibility after the initial term. Confirm later charges, notice and early-exit/refund terms before paying.
Follow-up and responsibility
Advertises 15 scheduled visits across the medical provider, clinical team and coach, plus app/community and group coaching. Not 15 physician appointments.
Still needs confirmation
State licensure, suitability, medication/lab charges, maintenance price and exact coordination responsibilities need written confirmation.

App and behavioral support

WeightWatchers

GLP-1 Success tools alongside an existing prescription. Paid registered-dietitian visits are a separate service; do not assume they are included in app membership.

Published costs
Published member self-pay RD options: one 30-minute visit $99; three $267; six $414; twelve $588. Membership is additional; its applicable current price is unconfirmed here.
Commitment and cancellation
Purchased RD visits expire after 12 months or when you unsubscribe from WeightWatchers. Check the separate membership commitment, renewal and refund terms.
Follow-up and responsibility
App and behavior resources; individual RD appointments available. A coach is not the same as a licensed clinician or registered dietitian.
Still needs confirmation
Your plan tier, insurance benefits, individual appointment access and prescriber communication need confirmation. Med+ prices are not companion-only prices.

A connected care team

Know who owns the next step.

An insurance change, a new provider or an additional care team can leave responsibilities unclear. Prepare the handoff without putting your records into another website.

Coverage or access changes

Collect the written decision, clarify deadlines and identify a clinical contact for an interruption. Coverage is never guaranteed.

Read the refill questions

Changing providers

Compare assessment, record-transfer, fees, cancellation and follow-up questions before assuming your prescription will continue.

Read the transfer checklist

Primary, nutrition and sleep care

Ask who coordinates results, referrals and records. For existing sleep-apnea care, significant weight changes can be a reason to ask the sleep clinician about reassessment.

Explore care coordination
Prepare a continuity checklist

Sleep testing and treatment changes belong to your clinician; do not stop PAP therapy based on weight change alone. NHLBI follow-up guidance. This is care navigation, not a new sleep-clinic service.

Evidence without promises

Maintenance results need context.

Withdrawal studies show why follow-up matters. They do not establish that a particular diet, app or off-ramp service will preserve someone's weight loss.

Read the maintenance evidencePlanning to stop or pause?

Before trusting a results claim

  • Was medication continued or stopped?
  • Was there a comparison group?
  • Were all participants counted, or only active users?
  • How long were people followed?
  • Does the claim describe this exact service?

Sources and editorial limits

2025 joint advisory: nutritional priorities during GLP-1 therapy

Expert advisory; supports individualized nutrition and activity care, not a guaranteed maintenance outcome.

STEP 1 extension: semaglutide withdrawal

Exploratory off-treatment follow-up; medication and the trial lifestyle intervention ended.

SURMOUNT-4: continued tirzepatide versus withdrawal

Randomized withdrawal study in adults without diabetes; not a test of a commercial off-ramp program.
