A little preparation. A better conversation.

Prepare your next care conversation.

Choose clinician questions or an administrative handoff checklist. Prepare for companion care, maintenance, coverage changes or a new provider. Clinical decisions remain with your licensed professionals.

Switching worksheets keeps your temporary choices. Reloading clears them.

1. Choose your research path

This selects question prompts. It does not assess your health or recommend a treatment change.

2. Add topics to discuss

Optional. Choose any that are useful.

Choices stay in this page's memory until you leave or reload. This tool does not save them to an account, send them to a provider or add them to analytics events. Normal page analytics are described in our privacy policy.

Your conversation, prepared

A useful sheet to bring to your care team.

Choose a path and add the topics you want to discuss. Your sheet will include focused questions, space for answers when printed, and links to the related guide.

01A named contact and clear responsibilities

02Questions about costs and follow-up

03A written plan from your own clinician

No symptom questionnaire. No weight or medication entries. No email gate.

When care crosses a boundary

Keep the handoff from becoming a gap.

Turn access problems, provider changes or a wider care team into a clear list of people to ask. No symptoms, records, medication entries or contact details needed.

Choose an administrative workflow

Prepare contacts, documents and questions. This does not determine appeal rights, guarantee approval or tell you how to use medication during a gap.

  1. Ask: Your prescriber

    1. Contact the prescribing team

Explain the access problem through the provider's secure channel. Ask for individualized written instructions and who to contact if treatment is interrupted. Do not change, ration or restart medication from this checklist.

  2. Ask: Insurer / plan administrator

    2. Get the exact reason and deadline

Request the written coverage decision, relevant benefit rule, appeal instructions and deadline. Ask whether the issue is authorization, a plan exclusion, an inactive policy, network status or another reason; different issues have different next steps.

  3. Ask: Pharmacy and provider billing team

    3. Confirm the dispensing and billing facts

Ask what can be filled, which prescription or authorization is pending and what any proposed cash charge includes. Ask who will communicate with the prescriber. Availability and a quoted price do not establish clinical appropriateness.

  4. Ask: You and your care team

    4. Organize a private document packet

Keep the decision letter, benefit details, provider contact information, relevant records and dates in your own secure location. Ask the clinician which supporting documents are appropriate. Do not upload these to GLP Clinic Finder.

  5. Ask: Named care-team contact

    5. Agree on the next contact

Confirm who follows up, by when, and how you receive the answer. If you explore another provider, confirm its assessment, transfer fees and care responsibilities before assuming a prescription will continue.

Changing the administrative workflow starts a fresh checklist. Copy your current output first if you want to keep it.
